Otoplasty Sculpture: Both ears conchal bowl setback, Hatch technique, and lobule setback.
45 year old male
Location Surgery: Bermant Plastic and Cosmetic Surgery Center (Office Operating Room)
Anesthesia: Local with sedation
The problem with this ear was that the conchal bowl was pushing the ear too far from the scalp. The main correction was setting the concha back further. There already was a good antihelical fold. To get the ear back far enough, extra tissue behind the ear had to be removed such that the concha was now sewn to the deeper structures just above the bone.

Moving the ear closer requires a balance between the various parts of the ear. In this case the top and bottom aspects did not move sufficiently without some additional help. The Hatch sutures permitted the upper part to be brought into harmony. The lobule setback brought the earlobe closer to the scalp. Failure to move these additional elements can result in a "telephone" ear deformity.
