Patient had a basal cell carcinoma of the lower eyelid. Excision was by modified Mohs' surgical control of margins (several sequential resections were needed until margins were clear of tumor). Reconstruction was by an advancement of the lower eyelid and cheek. The flap extended lateral to the eye allowing the remaining eyelid and cheek to move towards the nose. The lower drainage system of the eye (punctum) was spared and a tearing problem was avoided. Notice how nicely the eyelashes line up after surgery. Such detail is important for a good result.
