The neck lift is usually part of the facelift. Many patients have their neck muscles of facial expression (platysma) separated along the central aspect. When moving the SMAS layer back up to a position of more youthful appearance, the support for the repair starts here. Without a repair, separated platysmal muscles can spread further apart. This repair is done through a small chin incision.

The combination of SMAS elevation and repair of platysma muscles, helped redefine this patient's neck. The surgery of the neck is reached though incisions around the ears and through the tiny incision under the chin. When there is a good deal of extra skin, it needs to be removed as part of the lift. This placement of the incisions permits hiding the scars in facial details.

If you look at the sculpting of this neck as a result of:

• repair of platysma
• restoring the SMAS to a more youthful position higher in the face
• redraping the extra skin
• removing the excess through incisions camouflaged by facial features

you will begin to understand why a neck lift is best done as part of the face lift.
