This patient did not like his puffy nipples and protruding areola region. He wanted to achieve flat areola, more definition around his pectoral muscles, and a contoured athletic chest shape. The protrusion worsens when he flexed his chest muscles. He had been unable to achieve these goals with neither diet nor exercise.

Animation of the male chest, how it moves, is not easily shown with photos alone. Maintaining a layer of fat as what is found in adjacent areas improves the chance for natural movement of the superficial tissues. In the muscled male, there may be little fat to achieve this. Dr. Bermant's fat flap sculpture used adjacent tissue to fill the defect from this patient's gland excision.
