Gynecomastia Fat Flap Sculpting in the Muscled Male
This patient did not like his puffy nipples and protruding areola region. He wanted to achieve flat areola, more definition around his pectoral muscles, and a contoured athletic chest shape. The protrusion worsens when he flexed his chest muscles. He had been unable to achieve these goals with neither diet nor exercise.
25 year old male
5 foot 7 inches tall weighing 187 pounds
Location Surgery: Bermant Plastic and Cosmetic Surgery Center (Office Operating Room)
Anesthesia: Tumescent Local with Sedation
Method: Dynamic technique, excision mass (3.4 x 3.4 x 1.4 cm and 2.2 x 3.6 x 1.2 cm), liposuction contouring, and fat flap sculpture
Most of the deformity came from gland tissue. Lean muscular men can present a challenge for Gynecomastia reduction when there is a relatively large gland as in this case. Minimal fat can result in skin scared against the muscle and adhesions during movement. The glands were removed through the small incision at the lower areola / chest skin junction. In this patient, Dr. Bermant distributed the fat crater wall with a fat flap under the areola. Microcannular liposuction and sutures then finished the contouring.
