Patient competes in bodybuilding and did not like how his nipples were "poofy" at times and that there was a firm mass that tended to distort his chest during flexing. As a competitive bodybuilder, he had an extremely low body fat that tended to show the small glandular mass under his nipples.

It does not take much to distort the cut look of a bodybuilder. The firm gland tends to move and compress differently than fat and can look strange. The problem in sculpting this problem is not creating a crater defect with the gynecomastia surgery.
