Revision Gynecomastia Male Breast Reduction
This male patient stated he was unhappy with his chest after another doctor's breast reduction surgery done 10 months before. He complained that although the breasts were reduced, there seemed to be too much firm glandular tissue under the nipple and the areola region protruded. This resulted in an undesirable chest contour he did not like. The scar pulled on motion and he did not like the visible discolored scarring. This reduction had been by liposuction with gland excision through a wide incision around 180 degrees of each areola. The patient denied any breast growth after the first operation.

Anesthesia: Tumescent Local with Sedation
Method: Dynamic technique, revision gynecomastia recontouring with excision mass, microcannula liposuction, tissue sculpture, and scar excision.
Scars after another doctors male breast reduction were located around the lower half of the areola. Dr. Bermant only rarely extends the scar for gynecomastia surgery this far. His typical incisions are about only 2 cm compared to the 4 cm scars seen here. The areola scar was tethered and had thickened dark / light components.

The other doctor used 2 lower incisions on each side of the chest, something Dr. Bermant rarely needs for his patients sculpture. Such incisions can be more visible than one camouflaged at the areola skin junction.

It is easier to contour the chest initially with some fat to sculpt. Secondary gynecomastia surgery can be difficult with less fat to compensate for gland removed.

The natural male chest should move without tethering and not take on a more feminine contour as the skin is tightened over the muscles with movement. Dr. Bermant tries to design his gynecomastia chest sculpture with a consistent layer of fat found on the uninvolved chest. After surgery, there may be too little fat to compensate for further gland removal.
