Secondary male breast surgery must be individualized to the specific problem of that side of the male chest. The other surgeon left little fat on this side. Dr. Bermant normally only uses a 2 cm incision for his gynecomastia surgery. The unsightly scars from the prior surgeon forced the wider excision.

Dr. Bermant's has left extra gland tissue on the areola anticipating an eventual deficit. It is easier to take away than put back. After the gland excision, the underlying muscle can be seen in the wound.

Temporary sutures permit a determination on what extra fat tissue remains. Microcannula liposuction gave a precision to this fat sculpture.

No extra fat to approximate under areola means less natural movement |
Now that the final thickness has been determined, extra gland thickness can be trimmed. Closure incorporated the gland to the surrounding fat. An alternative choice might have been fat grafts from another region. These are less reliable than local fat flaps.
