28 year old male was embarrassed by his breasts. He wanted "to be able to be seen in public with my shirt off" and felt he was "missing out on life." He compensated by wearing thicker clothing, extra layers of clothing, and avoiding chest exposure. He did not like his chest shape, breast fat, puffy nipples, and sagging breasts ( Breast Ptosis.) He wanted to avoid more extensive skin reduction scars if possible.

Notice how the sagging chest tissues resulted in the areola / nipples being below the lower edge of the pectoral muscles before surgery.
