Photographs before donut mastopexy surgery to reduce big areola in male. Dr. Bermant tightens the chest skin using a permanent "purse-string" suture bringing the outer circle down to the desired size for the areola. The socket is a template for the new areola size.

The tension of the permanent suture has now reduced the chest skin outer wound. The length of the outer wound is much longer than the smaller areola resulting in wrinkling. The degree of wrinkling depends on the difference in the two lengths being approximated. Deep sutures balance the excess tissue about the new areola. Dr. Bermant then finely approximates the surface skin together with dissolvable sutures.

Skin tensions try to pull the areola larger. Without a force (permanent suture) to keep the areola size small, this tension stretches the areola. This is the complication from the patient's first skin reduction. The areola just stretched out until it became huge.
