Basal cell cancer of nose was removed with Modified Mohs' frozen section control of the margins. The resultant defect was reconstructed with a flap from the grove between the nose and the lip = the nasolabial fold. For larger defects, the nasolabial region can provide for a larger flap. The donor scar hides in the line between the nose and corner of the mouth.

The design of the flap is shown in blue. The defect size was determined by removing tissue until the pathologists confirmed that the margins had no more tumor (Modified Mohs' Surgery). When tissue is rotated in such plastic and reconstructive surgery, the skin folds into a deformity. This tissue is three dimensional. Over the next several months this tissue usually flattens out.
