Ethnic Nose Job or Rhinoplasty in a Man with a Wide Nose and Prior Nasal Fracture

42 year old male who did not like the way his nose dominated his face. He complained about his wide nose and that it was too big. The nostril and nasal tip width bothered him the most. There also was a bone deformity after nasal fracture 20 years earlier.

An o ld broken nose and nasal deformity must be part of planning for nasal sculpture. There was a depression and distortion of the nasal bones causing asymmetry and a palpable deformity. Narrowing the wide nasal bones must address the deformed old fracture.

CAT scans to the right are X-rays that are thin slices through the head as shown by the purple plane in the picture above. The disrupted nasal bones are in the top center of the scans.

Reduction of wide nasal bones can require sculpture of the medial bony structures.

After reducing the old nasal fracture, a wedge was removed from the dorsum so the bones could slide together. Moving the nasal bones together by the cheek will increase dorsal height. Too much elevation here can unbalance the nasal profile.

Narrowing a wide nose must blend the various elements to maintain harmony. The nasal bones were very wide in this male patient. These arrows point to the sculpture line between the nasal bones and the cheeks. Access was through tiny external osteotomy incisions - one on each side.

The wide upper lateral cartilage had to be narrowed. Some of the extra cartilage width was used instead of spreader grafts to open the nasal valves.
