Rhinoplasty Surgery Narrowing the Bones

External osteotomy (bone cuts) use a chisel to narrow the bone during rhinoplasty

This patient did not like the width of her nasal bones. External osteotomies (careful cutting of the nasal bones though the skin) divided the bones. Dr. Bermant is often able to narrow the nose though one tiny skin incision on each side.

There are many possible modifications of the nasal bones with rhinoplasty surgery. Techniques depend on the problem to be solved. Each patient's nose is treated as a unique surgical sculpture by Dr Bermant.

Here Dr. Bermant is using a specially sharpened 2 mm chisel to carefully cut the bone through a tiny incision on the nasal / cheek skin. There is a graphic sounding movie of this part of the operation here.

The advantage to the external approach is less damage to surrounding tissues during the sculpture and more precision in the bone cut.

With this patient, Dr. Bermant also divided the top part of the bone to narrow the nose. This part of the bony sculpture was done through the open rhinoplasty incision.

The bones now free to move can be adjusted to their new width. The nostrils have not been adjusted yet.
