Rhinoplasty for Caucasian Female with
Wide Nose and Nasal Tip
52 year old female who did not like her wide nose or bulbous tip and
wanted a smaller nose that better fit her face. Her skin was thick.

- Open rhinoplasty
- Slight dorsal reduction
- Narrow nasal bones
- Slight shortening of nose
- Spreader grafts and submucous resection septoplasty
- Narrowing and sculpture of nasal tip
- Columellar support cartilage graft
- Alar base wedge resection to narrow lower nose

The wide nose before surgery did not fit this patients face. Follow
the line from the nasal bridge to the tip of the nose in the before and
after surgery photographs. Examine the lines at the level along the juncture
of the nose and the face, and the lines at the top of the nose. Rhinoplasty
required reduction and blending of the various elements to produce a result
that better fit her face.

Heavy skin makes nasal sculpture difficult. Supporting structures are
forced to bear a greater force and fine detail can be obstructed. The
columellar cartilage graft helped support the narrowed nasal tip.

She had some difficulty breathing through both her nose. Narrowing a
nose can worsen nasal obstruction unless internal deformities are addressed.
In this patient, spreader grafts opened the constrictions along the roof
of the nasal passages. Septal deviations that would have obstructed the
nose were straightened. Other septal cartilage was used as a donor site
for the grafts used in the nasal sculpture.
