This patient decided against maxillofacial recontouring. She also did not want a chin advancement. She settled for orthognathic repositioning of her teeth and a septoplasty and rhinoplasty (septorhinoplasty). Nasal surgery should be planned after facial recontouring for the best result. Notice the improvement just with the hump removal with the rhinoplasty and the general contour change with the orthognathic manipulation. Her breathing problems were treated with the septoplasty

Female patient who had difficulty breathing through the nose. A deviated septum not only can block some of the airway, but also deflect the whole nose. Correcting the septum to an abnormal nasal structure is not very effective. Combining a rhinoplasty and septoplasty enabled the patient to breathe easily through her nose. Both the outside and inside deformities must be addressed. Restoring a bent tip to the midline can be very difficult.
