The Black Ethnic Nose Rhinoplasty Surgery
Removal
of the dorsal hump (bump on the top of the nose) required lowering the
deformity of both cartilage and bone. This was done through an open rhinoplasty. An incision was made through the
skin of the columella (post of tissue between the nostrils. The cuts continued
inside the nose. Tissue was then elevated just above the cartilages. A
series of rasps (very sharp surgical files) then removed the extra bone
and cartilage. Scissors and scalpels than finely trimmed the remaining
extra cartilage. I sometimes also use some very sharp chisels to perform
this surgical sculpture. You can see a movie

The depression between the nasal tip and hump turned out to be
accentuated by thicker than normal nasal transverse muscles. These
muscles needed to be surgically released to permit the skin to naturally
redrape. This unusual step is not normally necessary. Rhinoplasty
sculpture may require many techniques including intraoperative improvisation.

I also refined the nasal tip by narrowing very slightly the width of
the lower lateral cartilages. These cartilages hold up the lowest parts
of the nose the ala. The reduction can improve the gradual roundness of
the tip.

To reduce the width of the nose, the bones and attached
cartilages needed to be moved medially (inward). A balance must
be maintained between the upper and lower parts of the nose. An
alar base and nostril sill skin resection moved the lower nose together. |

After Rhinoplasty

Seen from below the changes are dramatic. As the interalar width was
reduced, the tip came up. The entire dorsum was designed to come up together.
The upper part of the nose moved with the bone infractures.
