New York Gynecomastia Revision for Liposuction Only Failure

Hello Dr. Bermant,

I was browsing on your website and came across this feedback (the 1st one) from one of your patients in NY .

It's very unfortunate that I read this now *After* I already had my gynecomastia surgery done by the same doctor - Dr.- in NY. It has been a little over 3 weeks since I've had the breast gland removed from my chest, however as your patient mentioned I still have puffiness in my nipples. Just as your patient explained, Dr.- is telling me to "give it some time" also. Although my chest certainly is flatter, the nipples though are still puffy and are slightly visible when I wear a T-shirt. This is the main reason I went for this surgery so that I can walk around without having to worry about the chest appearance.

Do you think 3 weeks is sufficient time for accurate results? I do feel some slight tenderness, but the pain isn't excruciating. I didn't have to take any pain killers post surgery either.

I definitely regret not coming across your site earlier or I certainly would have made the trip to VA. The "premium" cost as Dr.- stated was to receive quality service. Is it possible for me to receive an estimate from your office on the costs associated with removing the puffy appearance from my nipples? Are the costs usually lower in removing the puffiness than a regular full gynecomastia operation? Also, how much time do you think I should give it until I think about a 2nd surgery?

3 weeks is typically too soon to tell what the results will look like after surgery. How soon you can see surgical results after gynecomastia surgery depend on the problem to be treated, what was done, skill of the surgeon, after surgery care, and many other factors. It can take at least 6 months to see final results. For my patients, typical peak swelling after gynecomastia surgery is in the operating room and gradually decreases with time. Puffiness In Nipples After Gynecomastia Surgery can have many possible causes. Using liposuction first tends to target the fat, digging a hole for the gland. Some techniques also tend to cause more swelling and damage needing a longer time to see how tissues will evolve.

We need to understand the nature of your deformity to better discuss cost of revision surgery. The price can vary depending on what needs to be done. Yes, liposuction does not work on gland. Residual fat, scar, gland, and skin can remain behind as contour problems. Prices depend on what needs to be done. Hello -

I was hoping I could do a photo evaluation for a possible male chest lift / or nipple reduction. i had liposuction performed on my chest by a doctor in new york twice, most recent was done last April 07. T he second time was an attempt to even them out and get a better result from the first which was done June 06. I am still very unhappy with how I look in t shirts and although my chest muscle naturally is low set, ever since the procedure, my nipples appear to stick out more and are constantly hard. Not sure if I would be a candidate because I am thin and do not have much excess fat around my chest it is mainly muscle..
My main concern is scarring. I have two very small incision scars on my chest from the procedures - the doctor went into the same incision spots. I do not want additional scaring at all and wonder if it would be worth it.

I wonder if I can do a photo evaluation and possible follow up with an office visit, would there be an additional charge for the follow up office visit after the fee for photo evaluation?

Residual Puffy Nipple Complication After Gynecomastia Surgery can have many causes. Removing fat with liposuction can unmask the remaining gland that liposuction does not remove. Scar tissue can be as firm and deforming as gland. Revision Gynecomastia options depend on the original problem, what was done, and many other factors. It is easier to take away than replace.

I am a 46 yr old male and am interested in your Revision gynecomastia surgery. 2 years ago I had gynecomastia cosmetic surgery where the Dr only performed liposuction, and I feel I have a mild case of "puffy nipples" left over from it. I am primarily interested in reducing areola size and removing the puffiness around the nipple area. From your informative website I am also concerned w/ avoiding "crater deformity".

I have recently been diagnosed w/ Graves disease, for which I am taking medication for the thyroid, and am taking high blood pressure medication, but am otherwise in good health.

Revision Gynecomastia Surgery is an art form. I prefer my Dynamic Technique to minimize issues such as Crater Deformity. Other medical conditions sometimes need medical clearance before elective cosmetic surgery. As long as the blood pressure is under control, most patients with Graves disease can have this operation.

Hi Dr, Bermant my name is D I live in Yonkers New York. I suffer from gyno. About two years ago I went to the doctors and he recommended liposuction instead of getting the glands removed. It looked better but I was expecting more. I am looking to get the gyno removed once and for all. I am thirty-five years old and am 5' 10" tall 220lbs. I have been lifting weights my whole life and haven't been comfortable removing my shirt since I was nineteen. Does the twenty eight hundred include the full surgery, which is removal of the gland? What is the operating room fee? Can I leave after surgery? How long does it take to get an appointment? I look forward to hearing from you. D

We see many patients unhappy after liposuction alone surgery done elsewhere. Residual fat and gland can continue to cause contour issues. It does not take much Gynecomastia on a Bodybuilder to ruin a cut contour.

Cost of Gynecomastia Surgery depends on the problem to be treated. Revision Surgery is often more complex and therefore more expensive. For some years now I've had the problem with gaining access fat in the chest area. Although I have strong check muscles, there is excess fat around the nipple area. As noted in your website, I to cannot walk around with my shirt off; I try to wear a certain type of shirt as to not show my chest. When working out I hate to do exercises that prompt me to raise my arms high, as that is when my niple area really shows.

About one year ago, I have lipo in the chest area and under each lateral side of the arm. It realy appears the Dr. did not have the expertise to correct the chest area. While in the Army serving in Iraq, I hoped the exessive fat would burn off do to how hot it was and the fact we still had to exercise when time allowed. Still, I'm faced with the problem. I have had two other consults with Dr.'s here in NY and NJ. However, neither presents themselves as being confident about the outlook if they do the surgery. After reading and reviewing your site, to include looking at the before and after pics, I'm very impressed! The after photos really look good.

Dear Dr. Bermant:

My son developed gynecomastia at age 11. He is currently approaching 15 years of age. In June, my son underwent bi-lateral liposuction. He adhered to all the post-op instructions to include wearing the compression garment for three weeks. Unfortunately, the procedure that was utilized was completely unsuccessful, leaving my son in the identical state he was in prior to the surgery. Needless to say, my child is devastated by the outcome, and is still suffering from deep shame and humiliation. He is becoming reclusive, and has expressed a refusal to participate in mandatory swimming in gym. My son, D, has always been an honor student, an athlete, and popular with his peers. Watching my son socially isolate himself this summer has had a crippling impact.

As you well know, the insurance companies consider this a cosmetic procedure that does not fall into a "medical necessity" category (which I am appealing). I am a single mom, struggling to provide my son with a normal existence. An additional procedure, which I assume will entail cutting around the areola and removing glandular tissue, is probably indicated.

I was impressed with your site, and only wish I can make the trip to see you. We live in Suffolk County, Long Island, New York. If you can give me a referral to someone local with expertise in this area, I will be so very grateful.

Any advice you may be willing to give shall be appreciated.

Yes, there is often a major Emotional Impact Living with Gynecomastia. Liposuction does not manage gland. I use a tiny incision at the edge of the areola to remove gland. This scar hides well in the interface between the pigment of the areola and the chest skin.

New New York Gynecomastia Revision for Liposuction Only Failure

Dear Dr. Bermant,

I would like to inquire about surgery for gynecomastia. I have had a previous surgery about 2 years ago with a doctor in New York as I am located in New York City. The problem is that there seems to be a bit of glandular/fatty tissue left which causes the nipples to protrude (basically puffy nipples).

Although the surgeon did do a very good job in terms of no scarring and sculpting, I believe his idea of how the nipple should look differs from my idea (as he does a procedure for men who want more prominent nipples).

The normal male nipple areola complex should be a continuation of the natural muscle curve. Residual Puffy Nipple Complication can be a result of ignoring the gland element that is not addressed with lipo. New York Gynecomastia Revision for Liposuction Only Failure

Hi. I'm a 27 y/o weightlifter. I've had fat/glandular tissue around my chest for many years, and deccided to have surgery 6 months ago. My surgeon used a liposuction technique, with which he also removed most of the glandular tissue, but I still have some glandular tissue remaining. Even worse, removing the tissue has caused the area around the nipples to sag, leading to only minimal improvement. I would like a consulation with you based on the information on your web site. Unfortunately, I'm a medical resident so getting significant time off is next to impossible. If I were to start with a phone consultation and surgery seemed a reasonable option, would it be possible to do the in-person visit, surgery, and significant post-op care over a period of ~5 days, as even getting this amount of time off will be difficult? I'd be coming in from NY.

How well skin contracts with surgery depends on many factors. Leaving gland / scar tissue under the areola can impair shrinking.

Minimizing travel and time away from work is the hallmark of our Preliminary Remote Discussion. that starts out with pictures, detailed forms, and a telephone call. Our patients then can schedule a tentative surgery that still needs an in office confirmatory consultation (usually the day before surgery.) Stay after surgery depends on the problem to be treated. Many of our patients can leave Virginia the day after surgery.

I had a bi lateral Gynecomastia along with lipo to the abs and flanks done on June 24th by Dr - here in NYC. I saw - on 07/29 before leaving for vacation for a crease across the nipple of the right breast that he described as a "pinch point". He injected it with Cortisone and since I apparently have a rather high tolerance for pain told me to continue to stretch the area to see if it would release but if not when I returned make and appt and he could release it with just a local.......Upon my return I find that he is gone, the answering service stating that he has retired and only providing a fax number to request files, with a little bit of research I find that after over 30 years of quality practice his license has been revoked and he can no longer have interaction with patients......NOW, I am not looking (as far as I know ) for any further surgery, but the "pinch point" is still present and I can't say that I have achieved the results that I was looking for. All that being said, the few PS's that I have since called in my area that I had found during my initial investigation do not want to see me saying "you are now a re-do and we don't do re-do work", well I don't think I need to be redone but I have seen articles that indicates that you do in fact correct others mistakes. Again I can't fault Dr. - and can't say that this is a mistake as much as a result of how fast my body seems to heal, but I guess at this point I am asking for help without throwing more money out the window. I paid a lot of money for the procedure and at this point there is no one that is taking his patients or a referral so I figured I would come to the there anything that you can do to help me. Unfortunately I have seen many complications from that individual, someone who without training who decided to call himself a "plastic surgeon." From what I understand so many others also saw problems that eventually the government intervened and removed his license to practice medicine.

I have written several times in the past (my email used to be -) and have spoken to Dr. Bermant who indicated at the time that he had seen my problem and was well aware of my surgeon..............had bi-lateral gynecomastic surgery in 2005 with Dr. - and was one of his "last" patients before losing his license to practice (lucky me & my $12,000.00)............the post surgical complication is one that I have seen on your site where the area around the nipple is inverted or sunken , obviously due to too much tissue being taken out and not enough time taken to reconstruct... anyway, I can't stand it anymore, as I am no more comfortable now than I was before the surgery. At this point I just want to repair what can be repaired and let you take a look. I can handle the flight and accomodations for the first visit (as I have a few questions and I think it would be easier in person to get a hands on feel of the issues) but the one question that I do not see addressed on the site (GREAT SITE BTW) you have any financing plans........I am a Police Officer and after the 12K hit I took on the first one and the present financial climate, I would want to weigh my options on financing. Sorry this was so long, please let me know

Unhappy After Surgery By New York Doctor Who Lost License

We have corresponded on several occasions. I am a Police Officer here in New Jersey and I had a Bi-lateral gynecomastic surgery with the late Dr.- in 2005 and (ooohhhh so lucky for me) I was one of his LAST patients before his license was revoked. The right side was the problem side and did not think that there was anything wrong with the left but he convinced me that this could reoccur on the left after the work on the right so why take the chance and just do both at the same time……..well $12,000.00 later I was left (on the right side)with an inverted nipple and an indentation that you could put your index finger in!!!! The left side looks fine , but then as far as I was concerned it looked fine in the first place. Bottom line is that after all that money, the surgery, the subsequent visits where he did some "in office" cutting to try and release what he called a "pinch point"... I was left with a chest that to this day I think looks as bad if not worse than it did before!!!!!!!!

I am ready at this point to come to Va and try to make this right if that is even possible (I guess you will determine that when and if I see you)... one of the big problems is that for 2 years I have been trying to find my medical files but to no avail. I have nothing from Dr. - so I have nothing to show you where I was, what it looked like, what was done, etc. Since his death, I have contacted everyone from the Detectives that handled the investigation to the NY State Investigators that revoked his license and I have ... nothing.

Please let me know if #1 this creates a problem for you, #2 if considering all of this you would be willing to work on me and most importantly #3 some sort of general time frame... will I have to make numerous trips to Va or could we (if we are going to ) do this all in one trip.

Sorry for the long ramble but I wanted to give you a little background and I guess give you some sense of my frustration, anger and sheer disappointment at my own stupidity for somehow not seeing what should have been so obvious (hindsight of course being 20/20)!!!! Yes, we have been corresponding since at least 2005. Unfortunately, some patients just cannot get records. Although that puts an analysis at a disadvantage, some doctors records are poor anyway. Unhappy After Two Gynecomastia Surgeries Done In New York


First off, congrats on having the most honest and informative PS website on the web. I am sure you have gained many satisfied clients just because you are so thorough with the information you provide on your techniques.

I am writing to you in a bit of dismay. I have had 2 gynecomastia surgeries by a doctor in NYC (Madison Ave. no less) and am still not happy with my chest appearance.

The first surgery involved lipo only. He also did my back and love handles at this time which turned out great.

The second procedure involved lipo and some short scar (inner chest). He made this decision while he was working on me to tighten my chest appearance.

In neither did he remove gland even though I asked him to. The inner anchor skin removal was also not enough to deal with my lose skin issue.

I am 31, 6' 4" and 255 pounds. Bodybuilder type who has had major mental trauma in seeing my physique get affected over the years by bad genetics and high estrogen.

I would like to finally end this once and for all and seeing you perform this technique on someone who is much worse off than me gives me hope:

Short Scar Breast Lift

How can I get this process started? I would like to see you in late February so I can heal before summer. I've attached some pics of me posted second surgery. You'll notice the incision scars which have healed well and also the lack of symmetry as well as hanging skin around the nipples.

Please advise.

Liposuction does not work well for gland nor loose skin. I work with many Bodybuilders with gynecomastia. Excess skin of the male chest comes in many different degrees. Removing that extra skin involves a compromise. I prefer a short a scar as possible for my skin reductions. However, once another surgeon has made a surface scar, that scar cannot be removed, only revised. The revision scar is often as long as the original, which is why picking your first surgeon is so important.
