Read almost any body building forum and there are threads about problems with "puffy nipples."
Puffy Nipples, Puffy Nips, Puffed Nipples, Puffed Nips are common nonmedical terms for Gynecomastia.
These phrases are vague and mean many different things to different people. Dr. Bermant has seen many different distortions of the chest that his patients have called "puffy nipples." A disruption of the male chest profile can take many forms. A gently rounded pectoral profile just says male. With so many different possible problems, Dr. Bermant prefers to use his dynamic technique to individualize the management of the many different deformities of puffy nipples.
The best way to understand what someone means by "puffy nipples" is to visualize the problem.
Since Dr. Bermant sees patients from around the world, standardized views, looking at the problem from different angles and dynamically can help.
The nipple is the central projection where ducts transmit milk from female breast glands. The areola is the pigmented region surrounding the nipple. Just under the areola is a thin muscle that, when stimulated tightens the areola and can push the nipple forward. Puffy nipples is a misnomer for a distortion that can involve the nipple, areola, and surrounding regions.
Relaxed areola muscles permit the shape of pointed gland and fat to ruin the pectoral contour. Trying to keep the areola muscle stimulated is just not practical.

Puffy Nipples can be localized fat and gland in a combination of areola and surrounding regions. In the oblique profile view (seen above), the contour often centers around the areola. From the front, highlights above and shadows under the projection mark the contour distortion.

Sometime puffy nipples have a behind the areola gland and fat component and an external nipple problem.

All forms of Liposuction preferentially remove fat over gland. Liposuction can remove fingers of fat between fingers of gland, condensing a firm gland / scar mass. This firmer tissue does not compress like fat and can puff the nipple areola region. Residual fat can also disrupt the male contour causing male puffy nipples.
