Complication of Liposuction Gynecomastia Surgery by Another Doctor with Bad Scars and Adhesion Deformity on Animation.
This patient developed gynecomastia as a teenager and was dissatisfied after liposuction only surgery 15 years ago. He does not like the contour distortion and deformity that becomes very obvious when flexing his chest muscles. He also does not like his puffy nipples. His breast size has been stable. He asked Dr. Bermant for smaller and less protruding nipple regions.

Dr. Bermant's Recommendation: When fingers of gland extend through fingers of fat, liposuction preferentially targets the fat and may not even remove significant gland. Residual gland, scar tissue, and or too little fat can leave adhesion deformities as seen here. Scar and gland adhesions can be excised, but adhesions often recur unless a buffer is present. Fat can be a buffer between the skin and underlying muscle. When adjacent fat remains, fat flap sculpture can mobilize that fat to fill in the cavities and act as a soft buffer between the skin and muscle. It is easier to remove than to replace tissue. Dr. Bermant sees many patients from around the world who have had prior surgery by other doctors. Although Dr. Bermant is often able to offer surgical revision, not all deformities have reasonable revision options.
Scars can be a function of the original problem, surgical technique, skill of the surgeon, aftercare, and how a patient heals. Dr. Bermant prefers to minimize such scars during the initial surgery using his dynamic technique, permitting what he finds during surgery to guide what sculpture is best. Optimizing the sculpture, leaving a layer of natural fat, minimizing tissue trauma are part of the art of surgery. Dr. Bermant used before and after surgery pictures with the tissues in motion to demonstrate the quality / nature of tissue, scars, swelling, and bruising. Tightening the chest muscles looks bad, the scarring and adhesion distorts the chest.

The scarring is best seen from the side as the muscle pulls the skin in when tightened.

The degree of deformity was different on the left and right sides. The cavity defect was limited.
